Getting Started in Therapy:

#1. Physician Referral.
You can ask your child’s doctor for a referral for a speech and language evaluation and therapy with Joan Togami. The insurance company requires a physician’s referral. The doctor’s office may fax the referral to 254-265-7504.

#2. Initial Evaluation.
Once we have received the referral from your child’s doctor, you will be contacted for scheduling the first appointment for the assessment with Mrs. Togami.

#3. Case History Form:
Please fill out the Patient History Form (See link below) and return prior to the Initial Evaluation.

#4. What to Bring:
For the initial evaluation bring: insurance card and any previous medical or educational reports (Speech and language evaluations, IEP, MBSS, Developmental Pediatrician’s report, GI report, etc.)

#5. Insurance Authorization:
If therapy is recommended at the conclusion of the initial evaluation, the evaluation report will be sent to your child’s insurance. Many insurances will cover speech therapy, and some insurances require pre-authorization. Once your insurance company verifies they will cover speech therapy services, you will be contacted to schedule therapy.
